When customizing the Start Menu, the menu will hit a limit of how "short" it will get. This leaves a lot of wasted space.

Win9x has My Computer above My Documents, with XP this was switched. You could download TweakUI to reverse this back to the 9x days, but not in Vista!

Allow assignment of explorer actions
It really annoys me that if I'm browsing pictures (and not going through Photo Gallery) that I have to take my hands off the keyboard to click "Slide Show".
There should be a way to assign keystrokes to these explorer actions. They can be disabled by default but there should be a way to enable and possibly change them on an individual basis.

Include POSIX-style command line tools as an installation option
It would be great if I didn't have to install Cygwin to get POSIX-style command line utilities like ls, sftp, ssh, grep, etc. How many of you guys that jump from platform to platform have typed ls at a DOS prompt?
I know they weren't written by MS, but come on, can't we all just get along? They're free, they're simple and some of them have been around for decades. At the very least make them an installation option.

ALT+TAB Program Listing Favors the Desktop
Currently, ALT+TABbing between Desktop and other apps is inconsistent. Desktop always shows up as LAST application on the first row. This is unexpected, since the "last" program usually shows up as the 2nd in the list, UNLESS Desktop was the (more...)
The 2nd application (ie. two tabs away) should ALWAYS be the last program selection, whether or not it's the desktop. Inconsistent behaviour is annoying.

Popularity based on the percentage of promotes
A suggestion with 200 promotes and 50 demotes does not actually mean it's popular, and one with 30 promotes and no demotes should be more popular, but probability its just less read.
Add a way to sort the submissions based on the percentage of promotes.
Of course, in order to avoid having those with 1 votes being counted as 100%, filter out those with less than 10 (or 20) votes when sorted this way.

Annoyance in Vista's Windows Explorer navigation

This would easily be remedied by adding an option to right click context on the address bar to open the folder in new window. Or better yet put the up arrow back on the navigation bar next to organize, views and burn

Forced to use default email account when sending email from office application on Vista Bus
When using Office 2007 in conjunction with Vista the following occurs. There is no issue with XP

Viruses get spread by creating infected executable files that look like Vista Folders, and can get annoying because of another UAC prompt.

Weird UAC Prompt Placements
UAC prompt doesn’t always appear in the same place. It could show up in the middle, on the right, or even on the left. If this is on purpose, then there should be an option to for the user to set a default position.
Allow the UAC prompt to appear always in a single position; that is also friendly to multiple monitor setups.
